Marco never tried to inherit Whitebeard's title. When his captain dies at Marineford, he pulls together the surviving crew for a retaliation strike against Blackbeard, the Payback War, that ends in a defeat so total it kills any talk of a comeback for good. After that loss, he effectively dissolves the crew's remaining ambition and retires to Whitebeard's home village, working there as the village doctor, using his Phoenix Fruit's regenerative flames to heal other people's wounds rather than absorb his own in a fight.
That same regenerative flame was first spent on someone he barely knew: right after the war, it's Marco who treats Luffy's near-fatal wounds on the wrecked battlefield, keeping him alive just long enough to be carried off Marineford by his allies. The figure attached to his name, 1,374,000,000 berries, hasn't moved since that retirement: it's a former fleet commander's bounty, frozen on a man who simply stopped sailing altogether. When Nekomamushi comes to recruit him for Onigashima, Marco initially refuses, since the village still needs him, before joining the raid briefly to back up the Straw Hats against Big Mom and Kaido.
Once second-in-command of the largest pirate fleet ever assembled, he is now one of the few men of that rank still carrying a nine-figure bounty with no captain, no active crew, and no stated ambition beyond running a small clinic on an island the government has never once managed to erase from the map.
